Introduction

Embryo transfer is one of the most critical steps in the in vitro fertilization (IVF) process. During this stage, a fertilized embryo is placed into the uterus with the goal of achieving implantation and pregnancy.

While embryo quality is essential, successful implantation also depends on the condition of the uterine environment. Factors such as uterine blood flow, endometrial receptivity, nervous system balance, and hormonal signaling all play a role.

Because of this, many patients explore integrative therapies that may support their body during IVF. One of the most commonly used approaches is acupuncture before embryo transfer.

Acupuncture, a key component of Traditional Chinese Medicine, has been used for centuries to support reproductive health. In recent years, it has been studied as a complementary therapy during IVF cycles.


Why Embryo Transfer Success Matters

Even when embryos are genetically normal, implantation is not guaranteed. Several factors influence whether implantation occurs:

  • uterine blood flow
  • endometrial thickness and receptivity
  • uterine contractions
  • hormonal balance

What Research Says About Acupuncture Before Embryo Transfer
The Paulus Study (2002)

One of the most well-known studies on acupuncture and IVF was conducted by Paulus et al. (2002) and published in Fertility and Sterility.

In this randomized controlled trial:

  • Patients received acupuncture 25 minutes before and after embryo transfer
  • Results were compared to IVF without acupuncture

The study found:

  • 42.5% pregnancy rate with acupuncture
  • 26.3% pregnancy rate without acupuncture

This study sparked significant interest in acupuncture as a supportive therapy during IVF.


Subsequent Research and Meta-Analyses

Since the Paulus study, multiple systematic reviews have evaluated acupuncture and IVF outcomes.

2019 meta-analysis published in BMJ Open examined 27 randomized controlled trials and found that acupuncture may improve clinical pregnancy rates, particularly when used throughout the IVF cycle.

Researchers also noted that acupuncture appears to be more effective when used consistently before IVF, rather than only on the day of embryo transfer.


Mixed Results and Interpretation

It is important to note that not all studies show the same results.

Some trials have found minimal or no difference in pregnancy rates, which may be due to:

  • differences in acupuncture protocols and providers
  • timing of treatment
  • patient populations
  • study design variability

Overall, the current evidence suggests acupuncture may provide benefit, particularly when used as part of a broader, integrative fertility plan.

Research led by Stener-Victorin et al. demonstrated that acupuncture can:

  • increase blood flow to the uterus

Improved circulation may help support:

  • oxygen delivery
  • implantation conditions

2. Reduced Uterine Contractions

Excessive uterine contractions during embryo transfer may interfere with implantation.

Some studies suggest acupuncture may help:

  • reduce uterine hypercontractility
  • promote a more stable uterine environment

IVF can be a stressful experience. Chronic stress activates the sympathetic nervous system, which may negatively impact reproductive function.

When Should You Get Acupuncture for Embryo Transfer?

Many fertility specialists and acupuncturists recommend acupuncture:

  • 2-4 weeks leading up to FET
  • immediately before transfer– we go on-site to Conceptions, Kindbody, & Denver IVF and can offer both pre and post FET. If you are at CCRM or Shady Grove, we recommend coming to our clinic the morning of your FET.
  • immediately after transfer– we only recommend this in cases where we can go on-site to offer acupuncture before and after your FET (Conceptions, Kindbody, Denver IVF).

However, research suggests the best results occur when acupuncture is started weeks to months before IVF.

Our typical recommendations at Acupuncture Denver:

  • 1–2 treatments per week leading up to IVF for 2-3 months (but starting anytime is great!)
  • continued treatment during ovarian stimulation- weekly or twice weekly
  • targeted treatments around embryo transfer- 1-2x weekly for 2-4 weeks up to FET + day of FET

Is Acupuncture Safe Before Embryo Transfer?

When performed by a licensed practitioner trained in fertility care (ABORM-certified at least, and experienced or recommended by your fertility clinic at best!), acupuncture is considered safe during IVF. Fertility acupuncturists use specialized protocols and avoid points that may stimulate uterine contractions. Always inform your fertility clinic about any complementary therapies you are using.

Does acupuncture before embryo transfer improve success rates?

Some studies suggest acupuncture before embryo transfer may improve pregnancy rates by supporting uterine blood flow and reducing stress. We have seen many patients who have undergone repeated implantation failures, chemical pregnancies, and recurrent pregnancy loss who have gone on to have successful embryo transfers and live births!


Should I get acupuncture the day of embryo transfer?

Many patients receive acupuncture immediately before and after embryo transfer as part of their IVF protocol. We recommend this if we can go on-site (Conceptions, Kindbody) or if your clinic provides on-site acupuncture for FET (Shady Grove), but for our patients who cannot get on-site, we recommend coming ONLY before the FET (CCRM– try to schedule in the morning prior to going to the fertility clinic and go home to relax after FET). Coming to Colorado for a CCRM IVF cycle? We see many out of town patients on the reg!


How many acupuncture sessions are needed for IVF?

Many practitioners recommend weekly treatments for several weeks or months before IVF, with additional treatments during the cycle. In terms of FET prep, we recommend 2x weekly for 2-4 weeks leading up to FET. If you have high dopplers (reduced uterine artery blood flow), we recommend 2x weekly for 4 weeks using an evidence-based electroacupuncture protocol that improves uterine artery blood flow.


Is acupuncture safe before embryo transfer?
